Definitions

Operational Metrics- Hospice

• Name: Name of Hospice Agency
  
• Annual Admits: # of admits of hospice agency within the last four full quarters
  
• Discharge: # of discharges from the agency within the last four full quarters
• 15 Days (Visits in the Last): Average number of visits of hospice agency personnel 15 days prior to death of patients
• 7 Days (Visits in the Last): Average number of visits of hospice agency personnel 7 days prior to death of patients
• 3 Days (Visits in the Last): Average number of visits of hospice agency personnel 3 days prior to death of patients
• RN (Last 7 Days before Discipline): Average number of visits by hospice agency RNs within last 7 days of hospice patients' life
• LPN (Last 7 Days before Discipline): Average number of visits by hospice agency LPNs within last 7 days of hospice patients' life
• HHA (Last 7 Days before Discipline): Average number of visits by hospice agency assistant within last 7 days of hospice patients' life
• MSW (Last 7 Days before Discipline): Average number visits by MSW within last 7 days of patients' life
• • Home (Location of Care): Percentage of admissions with discharge that received care at home
