Definitions:

Hospital Discharges

Home Health

• Discharges: The number of home health discharges associated to the entity within the last four full quarters.
• Admissions: The number of home health admissions associated to the entity within the last four full quarters.
• Conversion (%): As a %, the home health conversion ratio between the number of admissions and discharges. 

Hospice

• Discharges: The number of hospice discharges associated to the entity within the last four full quarters.
• Admissions: The number of hospice admissions associated to the entity within the last four full quarters.
• Conversion (%): As a %, the home health conversion ratio between the number of admissions and discharges. 

Notes:

• For patient privacy, the smallest number displayed in annual admits is 11.
• The Market Intelligence Data used in this program is derived largely from Medicare and is dependent on Medicare’s reporting. To determine the reporting period, please refer to the Market Data Reporting Periods article.
